Emergency Medical Authorization forms due ASAP

Emergency Medical Authorization forms were due last Friday, August 24th. Please complete the authorization online through the Miami Valley CTC website. The authorization can be completed in a few minutes. Students may submit the information by using their computers at school or home. If preferred, a parent may complete a paper copy of the form. The form is available from the office or clinic. Should you have questions please contact MVCTC School Nurse at 937-854-6261. Thank you for your quick attention to this task.
